St. Joseph's Home of Springfield, a licensed long term care facility, has an exciting opportunity for an experienced Registered Nurse to join our Nursing Administration Team as a Care Plan Coordinator.
This is a critical care role, directing, coordinating and evaluating the 24-hour plan of care for the residents in a resident-directed care model with a strong emphasis on quality of care and quality of life. Facilitates communication of resident needs and issues to the Care Plan team to ensure needs are met timely and consistently. This individual fosters positive change and teamwork with the goal of maximizing well-being for residents, families, and those who care for them.
Position is M-F and will participate in the nursing administration on-call rotation. Requirements: Active RN License in Illinois along with 3 years long term care experience and supervisory experience.St. Joseph's Home is a smoke and drug free workplace.
St. Joseph’s Home of Springfield, a licensed not-for-profit long term care facility, has immediate full-time openings on Days and Evenings for LPNs and RNs with experience working in long term care to join our nursing team. Position timings include 1st shift (6am-2:30pm; e/o weekend) and 2nd shift (2pm-10:30pm; e/o weekend), and 3rd shift (10pm-6:30am; e/o weekend).
