Care Plan Coordinator

St. Joseph's Home of Springfield, a licensed long term care facility, has an exciting opportunity for an experienced Registered Nurse to join our Nursing Administration Team as a Care Plan Coordinator.

This is a critical care role, directing, coordinating and evaluating the 24-hour plan of care for the residents in a resident-directed care model with a strong emphasis on quality of care and quality of life. Facilitates communication of resident needs and issues to the Care Plan team to ensure needs are met timely and consistently. This individual fosters positive change and teamwork with the goal of maximizing well-being for residents, families, and those who care for them.

Position is M-F and will participate in the nursing administration on-call rotation. Requirements: Active RN License in Illinois along with 3 years long term care experience and supervisory experience.St. Joseph's Home is a smoke and drug free workplace.

Director of Admissions

St. Joseph's Home of Springfield, a licensed long term care facility, has an opening for a Director of Admissions to join our Administration team! This role is responsible for the planning, organizing, implementing, evaluating, and directing of the overall operations of all admissions and marketing in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as directed by the Executive Director. The success of this position derives solely from the provider relationships that result in referrals of appropriate members from physicians, discharge planner, case managers, and other healthcare professionals. Requirements include: Bachelor's degree in a related field preferred (Business, Marketing, and Communications); At least 2 years of prior healthcare related sales experience; Knowledge of admissions processes, related compliance procedures and other regulatory requirements.
